DBS Checks
How to complete your DBS check for SparkFish
Thank you for volunteering to help with SparkFish. It is our policy to ask volunteers who work regularly within schools to have a DBS check (formerly CRB) for their work with SparkFish. This may duplicate checks you have with other organisations, but at present that is the procedure we must follow.
SparkFish has an arrangement with uCheck to process the application online as quickly as possible. We would be grateful if you could do this by following the procedure below and then arranging to bring your identity documents into the office for processing by our administrator.
If you prefer, you can complete the online application at our office. Please contact office@sparkfish.org.uk to make an appointment. You will need to bring all the information required, such as NI number, address and name changes for the last five years with dates, to this appointment.
1st Stage – Completing the application online
- Go to https://www.dbsassist.co.uk/ucheck/
- Click on DBS Application Form – Start Application
- Enter Organisation reference – SPAR9 (Capital Letters)
- Enter Password – sparnine
- Click Enter
- The first page to appear will be the Statement of Fair Processing which will need to be ticked at the bottom of the page to confirm agreement
- The blank application will then appear
- Complete the application, using the notes on the right-hand side of the screen for guidance, with the applicant’s details. You will need clearance as a Child Worker only and the position applied for will normally be Youth Activity Worker
- Click in the box to indicate consent
- Click on Submit
- Make a note of the unique form reference number
- Click Home
2nd Stage – Bring identity documents to SparkFish
When you bring your documents into the SparkFish office, the SparkFish Administrator will confirm the identity documents that have been seen to support the application, together with the level of check required. THREE forms of ID will be required e.g. birth certificate, passport and utility bill showing your current address. See below for the list of acceptable documents.
3rd Stage – uCheck
- uCheck will submit your application for processing and inform SparkFish of the results within 5 working days

ID Checking Process
You must be able to provide (Route One):
- One document from Group 1
- Two additional documents from either Group 1 or Group 2a or 2b
One document must verify your current address. Route One must always be attempted to be taken. If you are not able to provide documentation in Route One, please contact the SparkFish office.
Group 1 – Primary Trusted Identity Credentials
- Current valid Passport
- Biometric Residence Permit (UK)
- Current Driving Licence Photocard (UK, Isle of Man, Channel Islands and EEA) (Full or Provisional)
- From 8 June 2015, the paper counterpart to the photocard driving licence will not be valid and will no longer be issued by DVLA
- Birth Certificate (UK and Channel Islands) – issued within 12 months of birth;
- Full or short form acceptable including those issued by UK authorities overseas, such as Embassies, High Commissions and HM Forces. (Photocopies are not acceptable)
- Adoption Certificate (UK and Channel Islands)
Group 2a – Trusted Government/State Issued Documents
- Current Driving Licence Photocard (All countries outside the EEA – excluding Isle of Man and Channel Islands (Full or Provisional)
- Current Driving Licence Paper Version if issued before 1998 (UK, Isle of Man, Channel Islands and EEA)
- Birth Certificate (UK, Isle of Man and Channel Islands) – (issued after the time of birth by the General Register Office/relevant authority i.e. Registrars – Photocopies are not acceptable)
- Marriage/Civil Partnership Certificate (UK and Channel Islands)
- Immigration Document, Visa or Work Permit (Issued by a country outside the EEA.)
- Valid only for roles whereby the applicant is living and working out of the Uk
- Visa/Permit must relate to the non EEA country in which the role is based
- HM Forces ID Card (UK)
- Fire Arms Licence (UK, Isle of Man and Channel Islands)
Group 2b – Financial/Social History Documents
- Mortgage Statement (UK or EEA)** (Non-EEA statements must not be accepted)
- Bank/Building Society Statement (UK, Channel Islands or EEA)* (Non-EEA statements must not be accepted)
- Bank/Building Society Statement (Countries outside the EEA)* (Branch must be in the country where applicant lives and works)
- Bank/Building Society Account Opening Confirmation Letter (UK) *
- Credit Card Statement (UK or EEA)* (Non-EEA statements must not be accepted)
- Financial Statement (UK) ** – e.g. pension, endowment, ISA (UK)
- P45/P60 Statement **(UK & Channel Islands)
- Council Tax Statement (UK & Channel Islands) **
- Letter of sponsorship from future employment provider (Non-UK or non-EEA only – valid only for applicants residing outside of the UK at time of application) (Must still be valid)
- Utility Bill (UK)* – Gas, Electricity, Water, Phone (landline only)
- Benefit Statement (UK)* – e.g. Child Allowance, Pension
- Central of local government, government agency, or local council document giving entitlement, for example from the Department for Work and Pensions, the Employment Service, HMRC* (UK and Channel Islands)
- EEA National ID Card
- Irish Passport Card (Cannot be used with an Irish passport)
- Cards carrying the PASS accreditation logo (UK, Isle of Man and Channel Islands)
- Letter from Head Teacher or College Principal (16-19 year olds in full time education in the UK (only used in exceptional circumstances if other documents cannot be provided))
Please Note, if a document in the list of Financial/Social History Documents is:
- Denoted with * – it should be less than three months old
- Denoted with ** – it should be issued within the past 12 months
- Not denoted – it can be more than 12 months old
